See a website form land in TMJ Journey
A contact form, an appointment request, a TMJ pain quiz and a sleep quiz — wired up the way a practice website would wire them. Fill one in and watch the request it sends, what comes back, the lead in the practice's inbox and the quiz on the patient's chart.
Try it
Nothing you type here leaves your browser. This page builds the request and the screens itself and sends nothing anywhere.
Sample questions written for this demo — not a clinical screening tool. A practice's own quiz scores itself and sends the result.
The page posts its answers to one endpoint with the practice's publishable key. formType says which kind of form it is; on a quiz, every question is sent as its own field and the quiz sends its own score.
A lead is filed — never a patient record. The reply names the form type, and on a quiz lists the fields it kept as answers so whoever wires the form up can check nothing else landed there.
It arrives in Marketing → Leads, tagged with what kind of form it was. A quiz shows its score; staff open the answers with one click and can filter the list by form type.
What it gives the practice
Keep the forms you have
Build them in whatever the practice's website already uses. Field names are matched for you — First Name, first_name and fname are the same field — and anything unrecognised is kept, not dropped.
Quiz answers stay with the practice
Scores and answers are shown only to the practice's own staff. They are never added to the lead's notes and never shown to a marketing agency's login — a symptom questionnaire is health information.
The right follow-up for each form
A practice can point each kind of form at its own automatic follow-up — one for sleep-quiz takers, another for contact forms — and when a lead becomes a patient the quiz is filed on their chart.
